The Model of Change: Why Your Beliefs Shape Lasting Transformation

Change is rarely a single “lightbulb moment.” Whether it’s improving your health, advancing your career, or building stronger relationships, transformation is a process. One of the most powerful tools to understand this process is the Stages of Change Model—also known as the Transtheoretical Model—developed by James Prochaska and Carlo DiClemente in the late 1970s.

This model breaks change into six stages:
Pre-contemplation – No intention to change and often unaware of the problem.
Contemplation – Beginning to recognize the need for change, weighing pros and cons.
Preparation – Gathering resources, making small initial changes.
Action – Actively changing behaviours and environments.
Maintenance – Sustaining the new behaviours and preventing relapse.
Relapse – Returning to old habits, which is a normal part of the process.
Why Attitude and Beliefs Matter Most
Many people cycle between action, maintenance, and relapse. The common roadblock? They haven’t addressed the foundational beliefs driving their behaviour.
If you believe “I’m not enough” or “I can’t change,” your subconscious mind will resist progress—even if you have the right plan. This resistance is hardwired into us. For early humans, stepping outside the familiar could threaten survival. Today, this translates into fear of judgment, fear of failure, and fear of standing out.
By shifting core beliefs to “I am capable,” “I am valuable,” and “I deserve this change,” you give yourself the internal stability to sustain transformation. This is where hypnotherapy and other mindset-focused approaches can help—by rewiring old patterns and aligning your identity with your goals.
Progress Over Perfection
Relapse is not failure—it’s feedback. Every day offers a new opportunity to choose differently. Real growth comes from balancing self-compassion with accountability. Holding yourself to a higher value increases your commitment, but so does accepting that imperfection is part of being human.
Whether your goal is weight loss, improved confidence, or career advancement, success lies in combining the practical steps of the change model with the deep mindset shifts that keep you moving forward.
